I use this while tanking. 15% proc rate sounds about right; not sure what the "hidden cooldown" is, but it feels like 45 seconds (fastest back-to-back proc was 50 seconds). 1 proc on trash, 2 or more on boss fights. The "effect" is about a +50% increase in threat for the duration (15 seconds).
